The Individual Questionnaire is completed online and can be accessed through the Central Bank of Ireland’s Online Reporting System (ONR).
Access to the Online Reporting System
RFSPs who currently have access to the ONR will require access to the Fitness and Probity returns. If you do not currently have access to these returns, please email email@example.com.
For new financial service providers seeking authorisation or existing RFSPs seeking access to ONR for the first time, please contact the appropriate authorisation/supervision team in the Central Bank of Ireland. Completing and Submitting an Individual Questionnaire
Please refer to the Online Individual Questionnaire User Manual and the Individual Questionnaire Template below for further information.
The proposing entity will provide the applicant with a user name and password, which the applicant uses to log into ONR. Once logged in, the applicant will have the option to start a new Individual Questionnaire application.
The Individual Questionnaire is completed online by an applicant seeking to perform one or more PCFs. Once completed, the Individual Questionnaire is submitted by the applicant to the proposing entity. An appropriate PCF holder (as specified in the Online Individual Questionnaire User Manual below) in the proposing entity will review the Individual Questionnaire, complete a declaration on behalf of the proposing entity and submit the Individual Questionnaire application to the Central Bank of Ireland for assessment.
Where the applicant is applying for PCF-10 (a Sole Trader), they complete a mandatory declaration and submit their Individual Questionnaire directly to the Central Bank of Ireland for assessment.
The Individual Questionnaire comprises of the 13 sections listed below, some of which may not be applicable to an applicant, depending on their personal circumstances.

Section 9: Positions as an Executive/Non-Executive Director, Chairman, Manager or Financial Service Provider in any entity (financial or other) where the position has not been approved by a financial services regulator
