• Acquiring Transaction Notification Form - This form is to be completed in respect of an application for a proposed change in qualifying holding.
• Form(s) relevant to Life Insurance Undertakings Unclaimed Life Assurance - Certificate of Compliance. The proceeds of life assurance policies unclaimed for between 5 and 15 years must be paid over to the State. This certificate must be submitted on an annual basis to the Central Bank pursuant to The Unclaimed Life Assurance Policies Act 2003. A Return of Funds or a Nil Return must also be made to the National Treasury Management Agency by the 30th April each year.
